Some More Off-List Comments

I've received a few more comments for protocol fixes/improvements off-list.
I intend to send these to the IESG when they open the last call:

- Increase the length of the contact postal code field to 16 characters
because postal codes longer than 10 characters (the current limit) are
possible in some countries, such as Mexico and France.  Perhaps list
participants living or working in those countries can confirm?

- Add a child "reason" text element to the <result> element so that error
conditions can be explained more fully in a server-defined, human-readable
way.
